Work Environment

This is an onsite position primarily focused on home care, enabling our therapists to work closely with patients in their own environments. This setup not only allows for flexible scheduling but also helps to establish stronger connections with patients and their families. You will have the autonomy to design your workflow while being supported by a dedicated administrative team that manages scheduling and logistics.

Key Responsibilities

• Conduct thorough assessments of patients' physical conditions and mobility limitations through discussions and evaluations during home visits.
• Diagnose movement dysfunction by utilizing detailed clinical assessments and develop customized treatment plans that address each patient's unique needs.
• Instruct patients on effective therapeutic exercise techniques and their correct application in day-to-day activities to maximize recovery and enhance movement ability.
• Administer various modalities such as therapeutic massage and manual therapy to promote healing and alleviate pain.
• Support patients in learning how to use mobility aids and medical equipment, ensuring they feel confident and safe during their rehabilitation process.
• Document patient progress meticulously in our electronic health record system, tracking goals, responses to treatment, and subsequent adjustments to care plans.
• Offer guidance to patients and their families regarding ongoing in-home exercises and treatment options to ensure continued progress outside of therapy sessions.

Patient Population / Client Focus

Our client base includes children, adults, and senior patients facing a variety of physical ailments and conditions, including post-surgical rehabilitations, sports injuries, neurological disorders, and chronic pain syndromes. Each therapist will have the opportunity to specialize in certain populations while benefiting from a diverse caseload that enriches their professional experience.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited institution.
• A valid State License in Physical Therapy to practice in New Jersey.
• Strong written and verbal communication skills to effectively collaborate with patients and their families.
• Basic proficiency in word processing and computer applications for record-keeping and documentation.
• Adaptability and a proactive approach to managing patient care in dynamic home environments.

Preferred Experience

While we require a minimum of two years of experience in a relevant Physical Therapy setting, we also welcome candidates who possess a strong desire to learn and grow within the home care sector. Experience in working with diverse patient populations and familiarity with home care practices will be considered advantageous.

Compensation and Benefits
