Work Environment

This position is based in a home care setting within the Lincroft area, allowing you to bring rehabilitation directly to the patient. Key Responsibilities

• Conduct comprehensive evaluations by assessing patients' medical histories, performing physical assessments, and documenting findings to inform individualized treatment plans.
• Engage in meaningful discussions with patients and their families to explain evaluation results and collaboratively set therapy goals tailored to their needs.
• Implement evidence-based treatment protocols across a variety of conditions while adapting methods to reflect each patient's progress and evolving health status.
• Document patient care activities accurately and timely, ensuring a clear record of interventions, progress summaries, and discharge plans that align with family and healthcare provider communication.
• Monitor patient outcomes and adapt care plans as necessary, consulting with other healthcare professionals to optimize treatment trajectories.
• Participate in departmental meetings and community outreach initiatives while striving to exceed productivity expectations and maintain an organized schedule.
• Encourage professional development by serving as a mentor to students and pursuing continuing education opportunities that contribute to your own practice and our organizational knowledge base.

Patient Population / Client Focus

In this role, you will provide therapeutic services to a diverse patient population that includes individuals recovering from orthopedic surgeries, those managing chronic conditions, and patients rehabilitating after strokes or injuries. Your expertise will allow you to offer tailored support for each patient's unique needs, working to enhance their functionality and quality of life through personalized care strategies.

Qualifications

• A Doctorate degree in Physical Therapy (DPT) is required.
• A valid physical therapy license in the State of New Jersey is mandatory.
• Certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association is required.
• Strong fundamental knowledge of physical therapy principles and practices.

Preferred Experience

• While two years of prior physical therapy experience is preferred, new graduates with a strong academic foundation and a passion for patient-centered care are encouraged to apply.

Compensation and Benefits
