Full-Time Home Care Physical Therapist

Position Overview

We are seeking a dedicated and skilled Full-Time Physical Therapist to join our team at Stone Crest Physical Therapy in Red Bank, New Jersey. In this role, you will work primarily in a home care setting, where you will evaluate patients, develop personalized treatment plans, and administer therapeutic interventions to promote recovery and enhance their quality of life.

Work Environment

As a home care facility, our work environment is dynamic and requires flexibility and adaptability. You will provide care directly in patients' homes, offering an intimate and personalized approach to therapy. Each day will present new challenges as you work throughout the community, ensuring that patients receive the highest quality of care in a familiar setting. The schedule includes 40 hours of work per week with some variable weekend hours.

Key Responsibilities

• Examine, evaluate, and establish comprehensive treatment plans for patients encountering functional limitations or disabilities.
• Collaborate with healthcare providers to ensure a seamless continuum of care for patients.
• Administer therapeutic interventions and re-evaluate treatment progress regularly to achieve optimal patient outcomes.
• Maintain meticulous documentation of patient assessments, treatment plans, and progress in compliance with regulatory standards.
• Lead educational sessions for patients and families to promote understanding of treatment processes and rehabilitation goals.
• Participate in community outreach initiatives and quality improvement activities to enhance service delivery and patient care.
• Oversee and mentor support staff, ensuring effective collaboration within the interdisciplinary team.

Patient Population / Client Focus

Your role will focus on a diverse patient demographic, including individuals recovering from surgical procedures, managing chronic conditions, or rehabilitating from injuries. You will work closely with a variety of ages, cultures, and backgrounds, tailoring approaches to meet each patient's unique needs. This will involve working collaboratively with families and caregivers to support their loved one's rehabilitation journey.

Qualifications

• A Bachelor's Degree in Physical Therapy is required for this position.
• State licensure in Physical Therapy is mandatory to practice in New Jersey.
• Strong written and verbal communication skills are essential for effective patient interactions and documentation.
• Proficiency in computer applications, including electronic health record systems, to facilitate accurate record-keeping and service delivery.

Preferred Experience

While a minimum of 2 years of experience in physical therapy is required, candidates with prior experience in home health settings or community-based care environments will be given preference. Additionally, experience in developing treatment plans and collaborating with interdisciplinary teams can enhance your application.

Compensation and Benefits

Stone Crest Physical Therapy offers a competitive salary range of $100,000 to $120,000 annually, commensurate with experience and qualifications.
