Work Environment

This position is primarily onsite within the homes of our patients in Shrewsbury and the surrounding areas. Key Responsibilities

• Assess and evaluate patients with a variety of orthopedic conditions, including those recovering from surgery or managing sports-related injuries, ensuring a thorough understanding of their individual needs.
• Create and implement customized treatment plans that reflect each patient's specific goals and functional progress, ensuring that they align with best practice guidelines.
• Utilize advanced manual therapy techniques, therapeutic exercises, and functional training methods to achieve optimal patient outcomes and enhance mobility.
• Educate patients and their families on movement mechanics, effective home exercise programs, and strategies for preventing future injuries, promoting self-management and independence.
• Foster strong communication and collaboration with orthopedic physicians, nursing staff, and other healthcare professionals to ensure a holistic approach to patient care.
• Maintain comprehensive and accurate patient records within our Electronic Medical Records (EMR) system, ensuring all documentation meets regulatory standards.
• Continuously stay informed of new advancements in physical therapy practices, incorporating evidence-based methods into treatment protocols.

Patient Population / Client Focus

This position focuses primarily on elderly patients who require rehabilitation services for orthopedic issues, post-surgical recoveries, and other mobility challenges. Our aim is to enhance their quality of life through personalized care and evidence-based rehabilitation techniques. Understanding the unique needs of elderly patients, including their overall health and connectivity to the community, is crucial for success in this role.

Qualifications

• A valid Physical Therapy license in the state of New Jersey.
• A Bachelor's Degree in Physical Therapy, with a preference for candidates holding a Doctor of Physical Therapy (DPT) degree or its equivalent from an accredited institution.
• Strong communication and interpersonal skills to effectively interact with patients, families, and healthcare teams.
• The capability to create and adapt patient-specific treatment plans tailored to individual progress.
• A genuine passion for helping patients achieve meaningful recovery and maintaining a supportive outlook towards their rehabilitation journey.
• The ability to work independently as well as collaboratively in a team-oriented environment.

Preferred Experience

• Minimum of two years of professional experience as a Physical Therapist, ideally with familiarity in an Assisted Living Facility or similar environments.
• A background in geriatric physical therapy or knowledge of age-related challenges affecting mobility and rehabilitation.
