Position Overview

Refresh & Motion Physical Therapy is seeking a dedicated Full-Time Physical Therapist to join our dynamic team in Lincroft, New Jersey. In this role, you will provide exceptional care to patients within a home care setting, where you will implement individualized treatment plans aimed at enhancing mobility, alleviating pain, and restoring function. Work Environment

As a Physical Therapist in home care, you will work onsite in patients' homes, providing a unique opportunity to deliver personalized therapy that aligns with their daily lives. This role requires you to have a flexible schedule, primarily from Monday to Friday, allowing for adjustments based on patient needs. You will engage closely with patients and their families, fostering meaningful connections and providing care that meets individual preferences and circumstances.

Key Responsibilities

• Conduct comprehensive evaluations, including the review of medical history and performance of specific assessments to identify patients' needs.
• Develop and clearly communicate individualized treatment plans to patients and their families, ensuring active participation in goal-setting processes.
• Implement treatment protocols proficiently, adapting approaches based on clinical insights and evolving patient health conditions.
• Collaborate with interdisciplinary healthcare providers as necessary, documenting changes in patient conditions and adjusting care plans accordingly.
• Ensure thorough discharge planning and follow-up care, maintaining accurate and timely documentation of therapy activities, progress notes, and discharge summaries.
• Demonstrate strong time management skills to meet productivity expectations while maintaining an organized patient schedule.
• Participate in marketing initiatives that promote our services within the community, enhancing patient access and awareness of available therapies.
• Contribute positively to departmental goals and adhere to regulatory standards, continually seeking to enhance the quality of care provided.
• Pursue ongoing professional development and engage in clinical instruction of students to foster new talent in the field.
• Promote a patient-centered approach that ensures excellent customer experiences and efficient access to care.

Patient Population / Client Focus

You will work primarily with diverse patient populations, including individuals recovering from surgery, elderly patients with mobility challenges, and those managing chronic pain conditions. Your role will be pivotal in understanding their unique needs, limitations, and goals, allowing you to create tailored therapy programs that improve their quality of life.

Qualifications

• Must possess a Doctorate in Physical Therapy (DPT) from an accredited program.
• A valid Physical Therapy license in the state of New Jersey is required.
• Basic Life Support (BLS) certification from the American Heart Association is mandatory.
• Continuing education credits or courses related to the field are preferred to maintain professional development.
• Patient care experience is highly valued, but new graduates with a strong educational background are also encouraged to apply.

Preferred Experience

While two years of relevant experience is preferred, we welcome applicants at all experience levels who demonstrate a passion for providing outstanding care. Knowledge of home care practices and familiarity with physical therapy techniques are advantageous.
