Work Environment

As a Physical Therapist in a home care setting, you will engage with patients within the comfort of their own homes, allowing for tailored and personalized care plans. Your schedule will primarily run from Monday to Friday, providing flexibility to adapt your treatment strategies based on the needs and conditions of each patient. Key Responsibilities

• Create individualized evaluation plans by analyzing patient histories and conducting thorough assessments.
• Discuss evaluation findings and develop comprehensive treatment plans involving patients and their families in goal-setting discussions.
• Utilize evidence-based treatment protocols tailored to patient's conditions, adapting as necessary to address evolving healthcare dynamics.
• Collaborate with interdisciplinary teams, effectively documenting any changes in patient conditions that may necessitate modifications to care plans.
• Ensure accurate, timely documentation of patient encounters, including treatment protocols and discharge summaries, thereby maintaining a clear record of progress throughout the care process.
• Diligently manage your schedule, meeting productivity requirements and participating in marketing efforts that promote community health awareness.
• Contribute to departmental goals by complying with governmental and accreditation regulations, promoting best practices within your team.
• Engage in continuous education and mentoring, fostering the professional development of peers and students within the healthcare field.
• Enhance patient access to care by proactively addressing barriers and leveraging community resources.

Patient Population / Client Focus

This position will involve working with a diverse patient population, including individuals recovering from orthopedic procedures, sports injuries, chronic pain management, and other rehabilitation needs. You will provide nurturing, empathetic care to improve mobility, strength, and quality of life for patients across various demographics. Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited program.
• A Doctorate in Physical Therapy is highly encouraged but not mandatory.
• Current state license to practice as a Physical Therapist in New Jersey.
• Current certification in Basic Life Support (BLS) for Healthcare Providers through the American Heart Association.

Preferred Experience
