Work Environment

You will work onsite within the home care setting, allowing for a unique and personalized approach to rehabilitation. This environment fosters one-on-one interactions with patients, encouraging a strong therapeutic alliance. Our clinic operates Monday to Friday from 8:00 AM to 5:00 PM, ensuring a balanced work schedule that supports both professional and personal well-being.

Key Responsibilities

• Evaluate patients with functional limitations through comprehensive assessments and develop individualized treatment plans that reflect each patient's specific needs.

• Implement evidence-based treatment protocols, modifying approaches based on patient progress and evolving healthcare standards.

• Coordinate with other healthcare providers to optimize patient care, documenting all findings and updates to treatment plans as necessary.

• Ensure accurate and timely documentation of evaluation results, treatment sessions, and discharge summaries, creating a clear record of the patient's rehabilitation journey.

• Participate in facility meetings and training sessions to maintain knowledge of best practices and compliance with state and federal regulations.

• Proactively address barriers to patient access and engage in initiatives to enhance service delivery within our practice.

Patient Population / Client Focus

In this role, you will primarily work with patients suffering from orthopedic conditions, including post-surgical rehabilitation, sports injuries, and chronic pain management. Your expertise is crucial in guiding these individuals toward improved physical function, pain relief, and overall wellness. Our patient-centered approach positions you at the forefront of their recovery journey, where your skills directly impact their ability to return to daily activities and achieve a better quality of life.

Qualifications

• Doctorate of Physical Therapy (DPT) from an accredited institution.

• Current state licensure as a Physical Therapist (PT) granted by the Physical Therapy Examining Committee.

• A minimum of two years of experience in outpatient orthopedic settings is mandatory.

• Current CPR certification to ensure readiness in emergency situations.

Preferred Experience

Experience with customized care plans in a home care setting is highly desirable, as well as familiarity with electronic health record (EHR) systems to streamline documentation and patient management processes. Candidates who demonstrate proficiency in utilizing therapeutic modalities and exercise programs targeted at orthopedic recovery will be preferred.

Compensation and Benefits

We offer a competitive annual salary in the range of USD 100,000 - 120,000, commensurate with experience, alongside a comprehensive benefits package aimed at promoting the well-being of our staff.
