Work Environment

This position is primarily onsite in the patient's home, allowing for personalized and flexible therapy sessions that adapt to individual environments. Key Responsibilities

• Conduct thorough evaluations of patients' physical conditions in home settings, assessing their strength, range of motion, and functional abilities to develop effective treatment protocols.
• Create individualized treatment plans that employ a combination of therapeutic exercises, manual therapy techniques, and patient education to help clients achieve their rehabilitation goals.
• Implement and monitor treatment interventions and therapies to enhance mobility, reduce pain, and improve overall function.
• Provide education to patients, families, and caregivers on therapeutic techniques, assistive devices, and self-care practices that facilitate continued progress.
• Regularly document patients' progress, utilizing electronic health record systems for efficient analysis and tracking, and adjusting treatment goals as necessary.
• Collaborate with interdisciplinary teams, including physicians and nursing staff, to share treatment insights and synchronize care efforts that benefit patient outcomes.
• Stay current with advancements in physical therapy practices by attending workshops and participating in professional development activities.

Patient Population / Client Focus

The role will involve working with patients of all ages who are recovering from orthopedic conditions, including post-surgical rehabilitation, sports injuries, and degenerative diseases. The focus on home care allows our therapists to assess and treat patients in a familiar environment, adapting therapy techniques to the specific requirements of each patient's home setting. Qualifications

• A Doctorate in Physical Therapy from an accredited program is required.
• Current state licensure in Physical Therapy.

Preferred Experience

Compensation and Benefits
