Key Responsibilities

• Conduct comprehensive evaluations of patients' physical conditions, history, and individual needs in their own homes.
• Design and implement customized treatment plans that utilize a variety of techniques, including manual therapy, therapeutic exercises, and advanced modalities to alleviate pain and enhance mobility.
• Continuously monitor patient progress and adjust treatment strategies as necessary to ensure optimal recovery and functional outcomes.
• Educate patients, caregivers, and family members about treatment plans, rehabilitation processes, and preventive measures to support ongoing health.
• Recommend and fit patients with assistive devices, ensuring proper usage to enhance mobility and self-care.
• Maintain accurate and thorough documentation of patient treatment, progress, and adjustments to facilitate continuity of care.
• Engage in ongoing professional development to stay updated with the latest evidence-based practices in physical therapy.

Patient Population / Client Focus

In this role, you will work with a diverse range of patients, including children and adults recovering from orthopedic surgeries, sports injuries, and chronic illnesses. Your ability to adapt treatment techniques to meet the unique requirements of each patient will be crucial in driving positive health outcomes. Building strong, trusting relationships with patients and their families will be a key component of your successful practice.

Qualifications

• A Doctorate Degree in Physical Therapy from an accredited program.
• Current State License in Physical Therapy.
• Excellent written and verbal communication skills, with a particular emphasis on patient education.
• Proficiency in computer skills and the ability to utilize electronic health record systems effectively.
• An agile and adaptable attitude towards working in varied home environments.

Preferred Experience

• Prior experience in home health physical therapy is preferred but not mandatory, as we offer robust mentorship for new graduates.
• Experience with orthopedic conditions and a familiarity with assistive devices will be an asset.

Compensation and Benefits
