Rhythm & Motion Physical Therapy and Wellness is seeking a dedicated and licensed Orthopedic Physical Therapist to join our team as a Home Care Specialist in Fords, New Jersey. Work Environment

The work environment for this role is primarily onsite, as you will be providing home care services to patients throughout the community. Your daily schedule will typically run from 8:00 am to 5:00 pm, Monday through Friday. This dynamic setting allows you to develop meaningful relationships with your patients and witness directly how your interventions positively impact their lives.

Key Responsibilities

• Adhere to state practice acts and comply with all laws governing the practice of physical therapy while overseeing the supervision of assistants as required.

• Conduct thorough examinations and evaluations of patients presenting functional limitations and disabilities, utilizing an array of assessment tools and techniques to inform treatment planning.

• Develop and implement individualized treatment plans based on detailed initial evaluations, ensuring that each plan addresses the specific needs and goals of your patients.

• Utilize current evidence-based treatment protocols and modify them as necessary according to each patient's progress and response to therapy.

• Consistently document patient care activities, including evaluation results, treatment implementations, and progress notes to create a comprehensive narrative of each patient's therapeutic journey.

• Engage with interdisciplinary team members and consult regarding patient changes that may necessitate shifts in treatment plans or referrals to other specialties.

• Assist with discharge planning, ensuring thorough communication of patient progress and future care recommendations to both the patient and relevant healthcare personnel.

• Participate in regular facility meetings, contributing valuable insights towards improving operational processes and patient access to care.

• Stay current with corporate and clinical training requirements to maintain compliance with local, state, and federal regulations impacting physical therapy practices.

Patient Population / Client Focus

Your role will focus predominantly on orthopedic patients who require physical therapy for rehabilitation following surgery, injury, or illness. You will provide treatment modalities ranging from manual therapy techniques to functional mobility training, tailored to each patient's unique recovery goals. Your expertise and empathetic approach will help patients navigate their recovery journey with confidence.

Qualifications

• A Doctorate of Physical Therapy (DPT) from an accredited program.

• Current licensure as a Physical Therapist (PT) in the state of New Jersey, validated by the Physical Therapy Examining Committee.

• A minimum of two years of outpatient orthopedic experience, demonstrating proficiency in delivering patient-centered care.

• Valid CPR certification, ensuring readiness for emergency situations that may arise during patient treatment.

Preferred Experience

• Experience working in a home health setting is highly desirable, enhancing your ability to perform therapy in a patient-centric environment.

• Familiarity with electronic health record (EHR) systems, which will aid in efficient tracking and documentation of patient progress.

• Advanced skills in implementing therapeutic exercise programs and functional mobility training.

Compensation and Benefits

The salary for this position ranges from $100,000 to $120,000 annually, commensurate with experience. Additional benefits include:
