Work Environment

This position is primarily onsite, focusing on home care. Key Responsibilities

• Conduct comprehensive evaluations and develop personalized treatment plans for patients as referred by their physicians.
• Administer and implement therapeutic interventions, including manual therapy, exercise prescription, and modalities, ensuring alignment with the patients' individual plans of care.
• Provide patient education for ongoing rehabilitation and prevention strategies, empowering patients and their caregivers with the knowledge to take an active role in recovery.
• Document patient progress meticulously, meeting health compliance standards while adjusting treatment plans based on patient responsiveness and feedback.
• Establish strong relationships with patients to understand their goals and challenges, creating a supportive atmosphere that enhances motivation and trust.
• Demonstrate superior customer service and collaborate effectively with team members to maximize patient outcomes and operational efficiency.

Patient Population / Client Focus

The primary focus of this position is on orthopedic patients, although you may also have the opportunity to work with pediatric, neurologic, and sports medicine cases. The diverse spectrum of patient conditions you encounter will require a tailored approach, making your ability to adapt and innovate in treatment strategies critical to your success in this role.

Qualifications

• Graduate of a physical therapy program accredited by the American Physical Therapy Association (APTA) or the Commission on Accreditation in Physical Therapy Education (CAPTE).
• Active Doctorate in Physical Therapy (DPT) degree.
• Current license to practice physical therapy in the state of New Jersey.
• Basic Life Support (BLS) certification from the American Heart Association.
• Demonstrated written and verbal communication skills necessary for effective interaction with patients and colleagues.
• Proficiency with standard office software and electronic health record (EHR) systems to ensure accurate documentation and streamlined communication.

Preferred Experience

• Prior experience in home health care settings is highly desirable.
• Experience in utilizing therapeutic modalities and evidence-based practice guidelines.
• Experience working with multidisciplinary teams to enhance patient care outcomes.

Compensation and Benefits

This full-time position offers a competitive annual salary ranging from USD 100,000 to 120,000, reflective of experience and qualifications.
