Work Environment

In this onsite role, you will be working in the intimate settings of our clients' homes, enabling a personal connection that promotes effective therapy outcomes. As part of a supportive team, you will collaborate closely with other healthcare professionals while maintaining a flexible schedule of Monday through Friday. Key Responsibilities

• Conduct thorough evaluations of clients' physical capabilities and limitations using specific diagnostic tests related to muscle, nerve, joint, and functional abilities.
• Develop and implement individualized treatment plans designed to prevent disabilities, relieve pain, and restore mobility in accordance with physicians' prescriptions.
• Oversee and mentor Physical Therapy Assistants to ensure adherence to the treatment plans and promote best practices in patient care.
• Provide hands-on therapy modalities, including manual exercises, traction, and massage, while instructing clients and families on functional activities.
• Evaluate the home environment and recommend modifications or adaptations to enhance client independence during rehabilitation.
• Maintain accurate clinical records, including initial evaluations, progress notes, and discharge summaries, ensuring compliance with agency policies and legal standards.
• Participate in regular interdisciplinary team meetings to discuss client progress, adjust treatment plans, and coordinate care.
• Communicate effectively with clients, families, and referral sources, and provide education on physical therapy techniques and the use of assistive devices.
• Finalize documentation and necessary clinical orders in a timely manner following branch protocols.
• Support the development and updating of client care plans through ongoing assessments and functional reviews.

Qualifications

• A Doctorate of Physical Therapy (DPT) from an accredited program is required.

• At least two (2) years of relevant professional experience in a similar role.
• Current CPR certification is a necessity to ensure client safety.

Preferred Experience

• Previous experience in home healthcare or rehabilitation settings is highly desirable.
• Familiarity with OASIS documentation and compliance standards is a plus.

Compensation and Benefits

We offer a competitive salary ranging from $100,000 to $120,000 per year.
