Work Environment

This position is primarily onsite, providing care directly in patients' homes. Key Responsibilities

• Conduct thorough evaluations of patients referred by physicians, assessing physical impairments, functional limitations, and rehabilitation potential.
• Develop individualized treatment plans based on sound clinical judgment and your evaluation findings, with a focus on orthopedic conditions and postoperative recovery.
• Utilize a variety of therapeutic modalities and techniques, such as manual therapy, therapeutic exercise, and patient education, to achieve optimal outcomes in patient care.
• Demonstrate exemplary customer service by establishing strong rapport with patients and ensuring their comfort and trust during home visits.
• Collaborate effectively with a multidisciplinary team, contributing to case discussions and sharing insights to enhance patient recovery processes.
• Educate patients and their caregivers about recovery processes, ensuring they understand their treatment plans and active roles in achieving health goals.
• Meet minimum utilization goals and contribute to the overall business objectives of the organization.

Patient Population / Client Focus

Your caseload will primarily include patients with orthopedic needs, but you will also have opportunities to engage with pediatric, neurologic, and sports medicine cases. This variety will allow you to expand your expertise while providing compassionate, focused care to individuals of all ages experiencing injuries or debilitating conditions.

Qualifications

• Graduate of an APTA or CAPTE-accredited physical therapy program.
• Current state license in Physical Therapy.
• Basic Life Support Certification from the American Heart Association.
• Strong written and verbal communication skills, with the ability to build positive relationships with patients and their families.
• Proficient in word processing and computer skills to maintain accurate patient records and documentation.

Preferred Experience

• A Doctorate in Physical Therapy.
• Experience working in home health care or community-based settings.
• Demonstrated ability to exceed functional goals and empower patients through education and engagement strategies.

Compensation and Benefits
