Key Responsibilities

• Conduct thorough patient evaluations, reviewing medical histories and utilizing specialized assessments to develop tailored treatment plans for each individual.
• Communicate evaluation findings clearly to patients and their families, engaging them in setting achievable goals for their rehabilitation journey.
• Implement evidence-based treatment protocols that adapt to the patient's condition and response to therapy, demonstrating proficiency in a variety of therapeutic techniques.
• Consult with other healthcare professionals to ensure comprehensive care, documenting any changes in the patient's condition and adjusting treatment plans accordingly.
• Prepare and execute detailed discharge plans and follow-up strategies, ensuring all documentation accurately reflects the patient's progress, treatment milestones, and expected outcomes.
• Maintain an organized schedule that adheres to productivity expectations, actively participating in hospital and community outreach initiatives.
• Contribute to departmental goals by engaging in quality improvement projects and educational opportunities for both personal and team development.
• Serve as a mentor and clinical instructor, facilitating internships and educational experiences for aspiring physical therapists.
• Enhance patient access to services by identifying barriers and actively participating in community engagement strategies.

Patient Population / Client Focus

Qualifications

• Doctorate in Physical Therapy (DPT) from an accredited institution.
• Valid New Jersey Physical Therapist license in good standing.
• Proven experience with manual therapy techniques and exercise prescription tailored to orthopedic conditions.
• Strong organizational skills, with the ability to manage time effectively and document patient care consistently using our WebPT EMR Software.
• Commitment to maintaining a clean professional record, free from disciplinary action.

Preferred Experience

• Experience in home health care settings, with a strong understanding of the unique challenges and opportunities presented in this environment.
• Prior involvement in clinical education as a mentor or clinical instructor.
• Familiarity with evidence-based practice and contributions to research initiatives within physical therapy.

Compensation and Benefits

We offer a competitive salary ranging from $100,000 to $120,000 annually, commensurate with experience and qualifications.
