Work Environment

This position is primarily onsite within the patients' homes, allowing for personalized and flexible care plans that align closely with individual recovery journeys. You'll work a full-time schedule, typically Monday to Friday, consisting of approximately 40 hours each week. Key Responsibilities

• Evaluate and assess patients referred by physicians to develop tailored treatment plans that address specific orthopedic needs.
• Implement and monitor effective treatment strategies utilizing various therapeutic modalities to promote recovery and improve patients' mobility.
• Provide skilled and compassionate care directly within patients' homes, ensuring the application of techniques and procedures that align with the physician's outlined plan of care.
• Consult, educate, and guide patients and their caregivers on recovery protocols and therapeutic exercises that support sustained progress and independence.
• Maintain comprehensive documentation of patient care, treatment outcomes, and ongoing assessments to track their recovery journey.
• Participate in interdisciplinary team meetings, collaborating with other healthcare professionals to optimize patient outcomes through shared insights and strategies.
• Consistently meet established utilization goals and contribute to the operational success of the practice.

Patient Population / Client Focus

Your expert care will be directed towards a diverse array of patients, including those recovering from orthopedic surgeries, sports-related injuries, chronic pain conditions, neurological challenges, and more. Qualifications

• A Doctorate degree in Physical Therapy (DPT) from an accredited program.
• A valid State License in Physical Therapy to practice in New Jersey.
• Two years of clinical experience in physical therapy, particularly in orthopedics or home care settings.
• Basic Life Support Certification from the American Heart Association.
• Proficient communication skills, both written and verbal, essential for patient interaction and documentation.
• Computer literacy, including familiarity with electronic health records (EHR) and basic word processing.
• Demonstrated ability to cultivate strong, empathetic relationships with patients, guiding them toward exceeding their functional goals.
• A proactive attitude toward connecting positively with individuals and building rapport in the community.

Preferred Experience

• Experience in evidence-based treatment practices, specifically targeting orthopedic rehabilitation.
• Prior exposure to home healthcare environments, understanding the nuances and challenges of patient care in a home setting.
• Proven ability to work effectively within a multidisciplinary team to enhance patient outcomes.

Compensation and Benefits

The position offers a competitive salary ranging from $100,000 to $120,000 annually, complemented by a comprehensive benefits package.
