Work Environment

This position is primarily onsite, providing essential physical therapy services directly in patients' homes. You will become an integral part of the local community, working closely with individuals while experiencing a variety of healthcare scenarios firsthand. Key Responsibilities

• Conduct thorough evaluations by reviewing pertinent medical history and performing specialized assessments tailored to each patient's needs.
• Develop and communicate individualized treatment plans, ensuring that patients and their families understand the objectives and procedures involved in their care journey.
• Implement evidence-based treatment protocols tailored to orthopedic conditions, while monitoring patient progress and making adjustments as required.
• Collaborate with multidisciplinary teams, including physicians and other healthcare professionals, to ensure comprehensive care and effective rehabilitation strategies.
• Maintain accurate documentation of patient care activities, including evaluations, treatment plans, and progress notes, following all regulatory and accreditation standards.
• Facilitate discharge planning and ensure patients have the necessary follow-up care and resources post-therapy.
• Participate in marketing activities that promote the services we offer and engage with community health initiatives.
• Actively pursue continuing education opportunities to further enhance your clinical skills and knowledge, serving as a clinical instructor for students when applicable.

Patient Population / Client Focus

You will primarily work with patients recovering from orthopedic surgery, managing acute and chronic musculoskeletal conditions. Our client base includes adults of varying ages who require personalized rehabilitation plans to regain strength and mobility. You will make a significant impact on their recovery trajectories, helping them to achieve independence and improve their overall quality of life.

Qualifications

• A Bachelor's or Doctorate degree in Physical Therapy from an accredited program.
• A valid state license to practice as a Physical Therapist in New Jersey.
• Current certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.
• A strong commitment to patient-centered care and professional ethics.

Preferred Experience

While prior work experience in a clinical setting is not required, candidates with experience in home care or outpatient orthopedic rehabilitation will be viewed favorably. Demonstrated expertise in utilizing contemporary therapeutic modalities and technologies relevant to orthopedic rehabilitation will strengthen your application.
