Work Environment

The role of Physical Therapist at Access Control Physical Therapy & Wellness is dynamic, requiring you to work in the home care setting, which allows for direct interaction with patients in familiar environments. Our team's culture emphasizes collaboration, respect, and support, ensuring that our clinicians feel empowered to deliver the best possible care. You will be working Monday through Friday, from 8 AM to 5 PM, providing consistent care and establishing ongoing relationships with your patients.

Key Responsibilities

• Conduct thorough patient evaluations by reviewing relevant medical histories and performing specific assessments to determine individualized treatment plans.
• Implement evidence-based treatment protocols tailored to each patient's unique condition, guiding them through recovery while adapting strategies as necessary based on ongoing evaluations.
• Maintain comprehensive documentation, including initial evaluations, treatment progress notes, discharge planning, and outcomes assessment to ensure clear communication with patients and the healthcare team.
• Engage in discharge planning and develop follow-up care strategies, ensuring patients are well-prepared for ongoing management of their health after their therapy ends.
• Collaborate with healthcare professionals, such as physicians and other therapists, to facilitate a holistic approach to patient care, enhancing recovery outcomes.
• Communicate openly and respectfully with patients, families, and team members to nurture a supportive and engaging therapeutic relationship.
• Attend team meetings and share knowledge with colleagues to foster continuous improvement and innovation within the therapy practice.

Patient Population / Client Focus

The Physical Therapist will primarily work with diverse patients undergoing orthopedic rehabilitation, including individuals recovering from surgery, injuries, or chronic pain conditions. This role requires a compassionate approach to patient care, understanding that physical therapy extends beyond healing to support emotional well-being and independence in daily activities.

Qualifications

• Doctorate Degree in Physical Therapy (DPT) from an accredited institution.
• Valid State Licensure to practice as a Physical Therapist.
• Current CPR certification.

Preferred Experience

We require a minimum of two years of experience in a physical therapy setting, preferably with a focus on orthopedic cases and home health care. Familiarity with the latest therapeutic modalities and a proactive approach to patient engagement will be advantageous.

Compensation and Benefits
