Key Responsibilities

• Conduct comprehensive evaluations of patients in their home environments, assessing physical abilities, limitations, and rehabilitation goals.
• Develop and implement individualized treatment plans using evidence-based practices, including therapeutic exercises, manual therapy techniques, and advanced modalities to enhance recovery.
• Utilize specialized equipment and assistive devices, educating patients and caregivers on correct usage to facilitate mobility and improved self-care.
• Monitor and document patient progress meticulously, making adjustments to treatment plans as required based on ongoing assessments and feedback.
• Collaborate with multi-disciplinary teams including nurses, occupational therapists, and physicians to ensure coordinated and holistic care.
• Coach and mentor patients and their families on maintaining therapeutic exercises and lifestyle adjustments outside of therapy sessions.
• Engage in continuous professional development through workshops and networking to stay abreast of industry advancements and best practices in physical therapy.

Patient Population / Client Focus

This role mainly serves orthopedic patients recovering from various conditions, such as recent surgeries, traumatic injuries, and chronic pain. Our diverse client base ranges from children to elderly adults, each requiring a customized approach that considers their specific diagnoses, living conditions, and personal goals. The therapist's role as a trusted rehabilitative partner is crucial in helping patients navigate their healing journey.

Qualifications

• Bachelor's degree in Physical Therapy; a Doctorate in Physical Therapy is preferred.
• Current state license to practice physical therapy in New Jersey.
• Exceptional verbal and written communication skills, enabling effective interaction with patients and healthcare teams.
• Proficiency in computer skills and experience with electronic medical records for documentation.
• Demonstrated flexibility and adaptability to changing patient needs and schedules.

Preferred Experience

• Previous experience in home health or orthopedic physical therapy is a plus but not required; we value fresh perspectives and welcome new graduates.
• Experience in assessing and prescribing assistive devices is beneficial.

Compensation and Benefits
