Work Environment

This position is primarily onsite, where you will work directly in patients' homes, evaluating their needs and providing tailored therapy in a comfortable and familiar environment. Key Responsibilities

As a Physical Therapist at Nealy Fitness Rehab Center, your responsibilities will include:

• Directing and implementing physical therapy treatment plans aimed at preventing disability, alleviating pain, and enhancing muscle function.
• Conducting comprehensive evaluations of clients, using diagnostic tests to assess muscle, nerve, joint, and functional capabilities.
• Overseeing the work of physical therapy assistants to ensure the effective execution of treatment plans.
• Visiting patients at home to administer therapies as prescribed by their physician while adapting treatments to suit individual circumstances.
• Documenting patient progress in clinical records, including initial evaluations and discharge summaries, and participating in interdisciplinary team meetings to address treatment outcomes collaboratively.
• Providing recommendations for home environment modifications to enhance client independence and functioning.
• Instructing and motivating clients in therapeutic exercises and functional activities to promote rehabilitation.
• Updating care plans regularly based on observed changes in patient conditions and responses to therapy.
• Completing clinical orders and maintaining impeccable documentation in accordance with company policies.
• Providing education to clients and families about physical therapy techniques and proper usage of therapeutic equipment.

Qualifications

The ideal candidate will possess the following qualifications:

• A Bachelor's Degree in Physical Therapy from an accredited program.
• A current and valid Physical Therapy license in New Jersey, in good standing.
• Current CPR certification.
• A valid driver's license and automobile insurance.

Compensation and Benefits
