Deep Care Physical Therapy is actively seeking a dedicated and compassionate Licensed Physical Therapist to join our team, providing high-quality home care services. Work Environment

This position will be entirely onsite, with therapists visiting patients in their homes throughout Keansburg, NJ, and surrounding areas. Key Responsibilities

• Conduct thorough evaluations of patients as referred by their physicians to determine appropriate individualized treatment plans.
• Develop and implement effective therapy programs tailored to each patient's specific needs and goals.
• Utilize a variety of treatment modalities and techniques, including therapeutic exercises and manual therapies, to promote healing and functional improvement.
• Educate patients and their caregivers on exercises and techniques to continue progress outside of therapy sessions.
• Document patient progress accurately and timely, ensuring compliance with legal and ethical standards.
• Maintain effective communication with other healthcare professionals to coordinate comprehensive care and monitor patient outcomes.
• Foster a positive environment by delivering excellent customer service and contributing to overall team effectiveness.
• Meet and exceed established utilization and business objectives consistently.

Patient Population / Client Focus

You will have the opportunity to work with a diverse patient population, focusing on a variety of specialties including orthopedic, pediatric, neurologic, and sports medicine. Each patient presents a unique set of challenges, allowing you to apply your clinical judgment and creativity in developing individualized treatment approaches that enhance recovery.

Qualifications

• Doctorate degree in Physical Therapy (DPT) from an accredited program.
• Current license to practice physical therapy in the state of New Jersey.
• Certification in Basic Life Support (BLS) from the American Heart Association.
• Strong written and verbal communication skills, along with proficient computer skills necessary for documentation and scheduling.
• Ability to establish and nurture strong relationships with patients, empowering them to achieve their functional goals.

Preferred Experience

A minimum of two years of experience in the field of physical therapy is required, preferably in a home care setting. Additional certifications or specialties in areas related to your patient demographic will be considered an asset.
