Key Responsibilities

As a Home Care Physical Therapist, your responsibilities will include:

• Directing and implementing physical therapy treatment plans aimed at preventing disability and relieving pain.
• Conducting comprehensive evaluations of clients' physical capabilities and functional abilities through diagnostic tests to tailor interventions to maximize their performance.
• Overseeing the activities of physical therapy assistants, ensuring proper adherence to individualized care plans.
• Administering manual therapeutic exercises and assisting clients with functional activities, ensuring motivation and instruction for non-manual exercises.
• Fitting and adjusting prosthetic and orthotic devices, making recommendations to optimize patient mobility and functionality.
• Preparing and maintaining detailed clinical records, including initial evaluations, care plans, progress notes, and discharge summaries for each patient.
• Collaborating within an interdisciplinary team to coordinate comprehensive care and participate in case conferences to plan and evaluate therapeutic approaches effectively.
• Providing consultation and guidance to clients and families about their physical therapy programs and the use of any prescribed devices.
• Maintaining timely documentation and communicating changes in patient conditions to relevant healthcare team members.
• Completing on-site functional assessments that review and revise the plan of care, ensuring all actions align with community resource utilization and client needs.

Qualifications

To be considered for this role, candidates must possess the following qualifications:

• A Doctorate degree in Physical Therapy (DPT) from an accredited program.

• Current CPR certification.
• A valid driver's license and proof of automobile insurance.
• Excellent interpersonal communication and patient engagement skills.
• Ability to exhibit strong clinical judgement and make challenging decisions.

Preferred Experience
