Work Environment

The Physical Therapist will conduct evaluations and treatments in patients' homes, providing a comfortable and familiar setting for recovery. Key Responsibilities

• Conduct thorough assessments of patients upon arrival at their residence to understand their specific needs based on their medical history and current health status.
• Establish and implement individualized treatment plans that utilize a variety of therapeutic techniques, including exercise modalities, manual therapy, and assistive devices.
• Regularly monitor and document treatment outcomes, noting patient progress and making adjustments to care plans as needed to maximize recovery potential.
• Educate patients and their caregivers about treatment procedures, exercise techniques, and the use of any necessary assistive devices to empower them in their recovery journey.
• Participate in ongoing professional development through workshops and training while staying up-to-date with the latest physical therapy practices.
• Collaborate with other healthcare professionals in a multidisciplinary team to ensure comprehensive care is provided to each patient.

Patient Population / Client Focus

Your primary focus will be on patients recovering from surgery, injuries, and chronic illnesses, providing them with the tools and knowledge they need to regain independence. You will work with a diverse patient demographic, including children and older adults, ensuring that care is tailored to each individual's unique needs, capabilities, and lifestyle goals.

Qualifications

• Doctorate in Physical Therapy (DPT) from an accredited program.
• Current valid state license to practice as a Physical Therapist in New Jersey.
• Strong verbal and written communication skills to effectively interact with patients, families, and multidisciplinary team members.
• Proficient in word processing and comfortable with using clinical documentation systems.
• A flexible and adaptive approach to treatment, with the ability to manage multiple patient cases efficiently.

Preferred Experience

• While a minimum of two years of experience is preferred, we welcome applications from new graduates passionate about impacting patient care positively.
• Experience in home care settings is advantageous but not mandatory.

Compensation and Benefits
