Work Environment

This role is primarily onsite, allowing you to deliver direct care to patients in their homes. Key Responsibilities

• Conduct thorough evaluations of each patient's condition upon first visit to determine treatment needs.
• Design and implement comprehensive treatment plans that include exercise regimens, manual therapy techniques, and guidance on the use of assistive devices.
• Utilize a variety of therapeutic modalities to alleviate pain and improve patients' mobility and quality of life.
• Regularly monitor and document each patient's progress, adjusting treatment protocols as necessary to ensure the best outcomes.
• Perform routine reassessments to redefine therapeutic goals and develop new treatment strategies to meet the evolving needs of each patient.
• Educate patients and their families on rehabilitation techniques and self-management strategies to promote home-based recovery.
• Foster professional relationships by actively participating in team meetings and contributing to care coordination discussions.
• Maintain compliance with all regulatory and accreditation standards in home health care services.
• Engage in continuous education through workshops and professional development activities to remain at the forefront of physical therapy practice.

Patient Population / Client Focus

Your clientele will include a diverse range of individuals, from children to older adults, each facing different challenges related to rehabilitation. You will work with patients recovering from orthopedic injuries, neurological conditions, post-operative recovery, and chronic pain syndromes. This broad spectrum of cases allows you to implement your skills in a variety of scenarios, enhancing both your expertise and patient care quality.

Qualifications

• A Doctorate degree in Physical Therapy (DPT) is required.
• A valid state license in Physical Therapy is a must to practice in the state of New Jersey.
• Exceptional verbal and written communication skills, essential for effective patient interactions and documentation.
• Proficiency in word processing and computer skills for patient records management.
• A flexible approach to work, adapting to patients' varying needs and schedules.

Preferred Experience

• A minimum of two years of relevant experience in a similar setting is preferred, though we are open to exceptional new graduates.
• Experience in home health care or outpatient settings is a plus.

Compensation and Benefits

This position offers a competitive annual salary ranging from USD 100,000 to 120,000, accompanied by a generous benefits package.
