Position Overview

Key Responsibilities

• Provide comprehensive patient assessments to evaluate therapeutic, rehabilitative, and functional status.
• Develop and implement individualized care plans in coordination with the healthcare team, focusing on patient engagement and achievement of goals.
• Deliver evidence-based physical therapy interventions tailored to each patient's needs throughout the continuum of care.
• Utilize treatment modalities and therapeutic techniques that promote recovery and enhance patient functioning.
• Instruct patients, family members, and caregivers on the use of therapeutic exercises, appliances, and prosthetic devices to facilitate home-based recovery.
• Prioritize caseloads and effectively manage daily staffing needs to ensure patient care is delivered promptly.

Patient Population / Client Focus

As a Physical Therapist at Vitalize Physical Therapy, you will primarily work with a diverse patient population ranging from adults to pediatric individuals undergoing surgical and non-surgical rehabilitation. You will treat an array of conditions, including musculoskeletal injuries, post-operative recovery, and chronic pain management. Your expertise will be crucial in empowering patients and assisting them in regaining mobility and independence.

Qualifications

• Current licensure as a Physical Therapist in California.
• Doctor of Physical Therapy (DPT) degree from an accredited program.
• Basic Cardiac Life Support (BCLS) certification must be obtained within 30 days of employment.
• No prior experience is required; new graduates are encouraged to apply.

Preferred Experience

• Previous clinical internship or rotation experience in an outpatient or rehabilitation setting.
• Familiarity with electronic health record systems and documentation practices.
• Experience with patient education and family involvement in care processes.

Compensation and Benefits
