Work Environment

This Home Care Physical Therapist position is an onsite role where you will deliver therapy services directly in patients' homes. Key Responsibilities

• Assess and adjust therapeutic goals as needed, collaborating with patients to achieve optimal outcomes.
• Train patients and caregivers in the safe use of prescribed assistive devices and recommend home modifications to improve the patient's living conditions.
• Maintain up-to-date knowledge of physical therapy advancements through continuing education and professional associations.

Patient Population / Client Focus

As a Home Care Physical Therapist with Adirondack Health Care Physical Therapy, you will work with a diverse range of patients, including pediatric and adult populations across various diagnoses. Qualifications

• Bachelor's Degree in Physical Therapy from an accredited institution is required. A Doctorate of Physical Therapy (DPT) is preferred.
• Valid state license to practice physical therapy and current CPR certification.

• Proficient in basic computer skills for documentation and treatment tracking.
