Work Environment

This position is primarily onsite, involving in-home visits for patient assessments and treatment. Key Responsibilities

• Conduct comprehensive evaluations of patients including thorough reviews of medical history and specific assessments to guide treatment recommendations.
• Effectively communicate evaluation outcomes and treatment plans to patients and their families, ensuring their involvement in the goal-setting process.
• Implement evidence-based treatment protocols tailored to the individual needs of each patient, while continuously adapting to their changing health conditions.
• Collaborate with a multidisciplinary team, recognizing critical changes in patient status that may necessitate adjustments in care plans or professional consultations.
• Oversee discharge planning and follow-up care; maintain accurate and timely documentation to validate the patient's care journey—from evaluations through to discharge.
• Manage time effectively to meet productivity targets while ensuring high-quality patient interactions; uphold an organized schedule and punctuality for meetings.
• Participate in hospital and community outreach initiatives, promoting the services offered and increasing awareness of available care options.
• Contribute to department goals and quality improvement projects, aligning personal performance with our strategic mission and compliance standards.
• Engage in ongoing professional development; mentor students in clinical practice as needed, reinforcing your role as a leader in the field.
• Facilitate an exceptional patient experience by demonstrating flexibility and responsiveness to patient needs and preferences.

Patient Population / Client Focus

Qualifications

• A Doctorate degree in Physical Therapy (DPT) from an accredited institution.
• A valid Physical Therapist license in the state of New Jersey.
• Current certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.
• A strong commitment to continuing education and professional development.

Preferred Experience

• While prior work experience is preferred, candidates with a passion for patient care and a willingness to learn will also be considered.
