Work Environment

You will work primarily in home care settings, allowing you to create meaningful one-on-one interactions with patients in the comfort of their own homes. The standard working hours are Monday through Friday, from 8:00 AM to 5:00 PM, offering a balanced work-life structure.

Key Responsibilities

• Assess and analyze patient conditions by conducting thorough evaluations and developing tailored treatment plans that address functional limitations and disabilities.
• Implement evidence-based treatment protocols while monitoring progress and adapting plans as necessary based on patient responses and emerging healthcare trends.
• Maintain meticulous documentation of patient evaluations, treatment sessions, progress notes, and discharge summaries to support continuity of care and compliance with healthcare regulations.
• Collaborate with interdisciplinary healthcare teams to enhance patient outcomes and facilitate optimal care pathways.
• Conduct regular follow-ups and adjust treatment plans based on evaluated outcomes, ensuring clear communication with patients and their families about progress and expectations.
• Participate actively in facility meetings and contribute to initiatives aimed at improving patient access and care delivery.
• Stay informed of changes in state practice acts to ensure compliance in all aspects of care, supervision, and documentation.

Patient Population / Client Focus

As a Physical Therapist at Access Control Physical Therapy & Wellness, you will primarily serve adult patients experiencing orthopedic issues. Many patients referred for home care possess various mobility challenges due to post-operative rehabilitation, chronic pain conditions, or neurological impairments. Your expertise will significantly enhance their recovery journey, making daily activities more manageable and improving their overall quality of life.

Qualifications

• A Doctorate of Physical Therapy (DPT) from an accredited program.
• Current state licensure as a Physical Therapist.
• Two years of experience in outpatient orthopedic settings, with a focus on home care beneficial.
• Possession of a current CPR certification.

Preferred Experience

Experience working with geriatric patients and familiarity with home care operations will be highly regarded. Skills in patient education and therapeutic exercise strategies will also enhance your effectiveness in this role.
