Work Environment

As a physical therapist at Pike Creek, you will operate primarily in home care environments. Key Responsibilities

• Conduct comprehensive evaluations of patients referred by physicians to determine individualized treatment plans based on their specific conditions and rehabilitation goals.
• Implement and monitor treatment programs, employing advanced therapeutic techniques and evidence-based methodologies to promote recovery.
• Provide skilled therapeutic care in patients' homes using procedures that align with each patient's physician-approved plan of care.
• Educate and train patients and their caregivers on techniques, exercises, and equipment that promote recovery and enhance quality of life.
• Maintain meticulous patient records and documentation in compliance with legal and ethical standards to ensure seamless communication with referral sources and other healthcare providers.
• Achieve minimum utilization goals while contributing to the overall effectiveness and operational objectives of the organization.
• Collaborate with a multidisciplinary team to evaluate patient outcomes and continuously improve service delivery.

Patient Population / Client Focus

This role primarily serves a diverse patient population, including orthopedic, pediatric, neurologic, and sports medicine clients. The therapist's ability to adapt treatment strategies to meet the specific needs of each patient will be crucial to their success. Qualifications

• Graduation from an APTA or CAPTE-approved physical therapy program is mandatory.
• A Doctorate in Physical Therapy is required.
• Possession of a current and valid state license in Physical Therapy.
• Basic Life Support (BLS) Certification from the American Heart Association.
• Excellent oral and written communication skills, with proficiency in word processing and basic computer applications.
• A positive, engaging approach that fosters strong relationships with patients and caregivers.

Preferred Experience

• Experience in home care or community-based rehabilitation is highly desirable.
• Background in orthopedic therapy with a strong understanding of musculoskeletal conditions and recovery techniques.
• Experience with electronic medical records (EMR) systems to streamline documentation and patient communication.

Compensation and Benefits
