Work Environment

This position is primarily based in the homes of our clients in the Highland Park area. The ideal candidate must be comfortable and adaptable to various home environments, capable of assessing and modifying treatment plans based on the unique physical space and resources available to each patient. You will collaborate closely with an interdisciplinary team and ensure that the client's home is conducive to rehabilitation.

Key Responsibilities

• Develop and implement personalized treatment plans for patients that focus on pain relief, regaining mobility, and enhancing overall function, in accordance with individualized care goals established in conjunction with the client's physician.
• Oversee and guide Physical Therapy Assistants in delivering effective therapy services, ensuring adherence to the care plan and quality therapeutic techniques.
• Conduct thorough evaluations of each client's physical capabilities through diagnostic tests and personal assessments to create targeted and effective therapy plans.
• Evaluate the home environment to recommend modifications or enhancements that increase client independence and safety during rehabilitation.
• Administer evidence-based therapeutic exercises, including manual and non-manual techniques, and motivate clients to actively participate in their rehabilitation journey.
• Manage the application of physical modalities such as traction and massage, and document treatment effectiveness for ongoing improvements.
• Assess and adjust prosthetic and orthotic devices as needed, consulting with physicians regarding any changes in client needs.
• Maintain comprehensive clinical records, including initial evaluations, progress notes, and discharge summaries, ensuring timely and accurate documentation as per agency policy.
• Participate actively in interdisciplinary case conferences to foster a collaborative approach toward holistic client care.
• Instruct clients and their families in using therapy devices and techniques, ensuring an understanding of the client's overall therapy program.
• Supervise the Physical Therapy Assistant in accordance with state regulations and assist in their professional development by providing mentorship.
• Conduct functional assessments regularly, reviewing and revising care plans based on thorough evaluation and utilization of community resources.
• Complete any additional assigned duties that contribute to the goals and objectives of our organization.

Patient Population / Client Focus

In this role, you will work with a diverse patient population requiring orthopedic rehabilitation, including those recovering from surgery, injury, or chronic conditions. A strong commitment to addressing the individual needs of patients and their families is essential, as you will be performing your work in their homes, often requiring empathetic communication and problem-solving skills to navigate unique challenges.

Qualifications

• A graduate degree from an accredited physical therapy program.
• Current licensure as a Physical Therapist in the State of New Jersey with no disciplinary actions.
• At least one year of experience in a physical therapy role, with a preference for experience in acute care or home care settings.
• Current CPR certification and a valid driver's license with reliable transportation and insurance.
• A proactive mindset, capable of fostering creative solutions, while taking responsibility for clinical decisions.
• A passion for enhancing the quality of life for patients through innovative and tailored physical therapy treatments.

Preferred Experience

• Experience working in home health settings, particularly focusing on orthopedic conditions.
• Familiarity with documentation requirements such as OASIS and care plan development.

Compensation and Benefits

The salary for this position ranges from USD 100,000 to 120,000 annually, commensurate with experience and qualifications.
