Work Environment

This role is performed onsite in the home care environment, giving you the unique opportunity to work directly with patients in their own homes. Key Responsibilities

• Conduct thorough patient assessments in their home environment, evaluating their physical condition and identifying rehabilitation needs.
• Design and implement individualized treatment plans using diverse therapeutic techniques tailored to enhance patient mobility and reduce pain.
• Utilize methods such as therapeutic exercises, hands-on care, and equipment as necessary to facilitate patient recovery.
• Monitor and document patient progress meticulously, adjusting treatment plans as needed based on patient responses and outcomes.
• Guide and train patients and their families on the best practices for rehabilitation, including exercises and the use of assistive devices.
• Engage in continued education and professional development by attending workshops, conferences, and staying updated with current literature in the field.
• Collaborate closely with other healthcare professionals to ensure comprehensive care for each patient.

Patient Population / Client Focus

The primary focus of our home care Physical Therapists is to assist a diverse patient population, including children and adults of varying ages who are recovering from injuries, chronic conditions, surgical interventions, or other health-related issues. By understanding each patient's unique situation and personal goals, you will create an atmosphere of trust while motivating them on their path to recovery.

Qualifications

• Bachelor's Degree in Physical Therapy or a higher qualification such as a Doctorate of Physical Therapy (DPT) from an accredited program.
• A valid state license to practice as a Physical Therapist in New Jersey.
• Competent communication skills, both verbal and written, as well as proficiency in computer applications for documentation and reporting.
• Exceptional flexibility and an ability to adapt to diverse patient needs and environments.
• Current CPR certification to ensure patient safety at all times.

Preferred Experience

• A minimum of two years of work experience as a Physical Therapist is required, showcasing a solid foundation of clinical knowledge and skills.

Compensation and Benefits

We offer a competitive salary range of USD 100,000 to 120,000 annually, commensurate with experience, along with an attractive benefits package designed to support your overall well-being.
