Work Environment

This role requires on-site presence as a home care provider, where you will interact with patients within their living environments. It is structured for a full-time commitment of 40 scheduled hours per week, with variable weekends based on patient needs. Key Responsibilities

• Conduct thorough patient evaluations to identify functional limitations and develop individualized treatment plans.
• Implement and monitor progress on treatment programs, adjusting as required based on regular reassessments and patient feedback.
• Provide expert consultation to healthcare providers regarding the continuum of care for our patients.
• Document treatment outcomes meticulously, and manage billing processes accurately to reflect services delivered.
• Participate in community outreach initiatives that promote physical health and wellness.
• Engage in staff development sessions, including leading in-services that facilitate professional growth.
• Supervise and mentor support staff and students, fostering a learning environment and ensuring quality care.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited program.
