Work Environment

As a Home Health Physical Therapist, you will work onsite, providing care directly in the homes of our patients. The typical work schedule is Monday through Friday from 9 AM to 5 PM. This role allows for a rewarding blend of independence and support as you connect with patients where they feel most comfortable. You will work collaboratively with an interdisciplinary team that includes nursing, occupational therapy, and social work professionals to ensure coordinated and comprehensive care.

Key Responsibilities

• Conduct thorough in-home physical therapy evaluations to identify patient needs and develop customized treatment plans that address their specific health goals.
• Deliver a wide range of therapeutic interventions, including therapeutic exercises, gait training, balance exercises, and functional mobility training, tailored to each individual's needs.
• Educate patients and caregivers on vital home safety measures, fall prevention techniques, and overall wellness strategies to promote healthier living environments.
• Track patient progress meticulously, adjusting treatment plans as needed to align with each patient's functional goals and rehabilitation progress.
• Maintain accurate and timely documentation of all visits according to Medicare and agency guidelines, ensuring compliance with regulatory standards.
• Engage in effective communication with other members of the healthcare team to facilitate seamless patient care and coordination.

Patient Population / Client Focus

The patients you will serve may include individuals recovering from surgeries, managing chronic conditions, or rehabilitating after injuries. Our diverse patient population requires tailored approaches to care, focusing on orthopedic, neurological, and post-surgical recovery, fostering a supportive environment for achieving their rehabilitation goals.

Qualifications

• Active and valid New Jersey Physical Therapist License is required.
• A Bachelor's degree in Physical Therapy from an accredited program is essential.
• A valid driver's license, reliable vehicle, and auto insurance to facilitate in-home visits.
• Current CPR/BLS certification.
• Demonstrated strong clinical judgment and time-management skills to handle various patient needs efficiently.
• Ability to work independently in a homecare setting while delivering high-quality patient care.

Preferred Experience

• At least two years of professional experience within home health, outpatient, or hospital-based rehabilitation.
• Familiarity with electronic medical record systems (e.g., HomeCare HomeBase, Kinnser, MatrixCare) to enhance documentation processes.
• Knowledge of Medicare documentation requirements and OASIS (although training will be provided, experience is a plus).

Compensation and Benefits
