Work Environment

This position is primarily onsite, working directly in patients' homes across the Daytona Beach area. You will engage with patients in their familiar environment, which can significantly enhance their comfort levels and participation in the therapy process. Key Responsibilities

• Collaborate with a multidisciplinary team of healthcare professionals to develop comprehensive treatment plans based on each patient's unique medical history, needs, and therapy goals.
• Utilize evidence-based practices to assess and treat patients with varied conditions including orthopedic, neurological, and pediatric disorders, ensuring delivery of the highest standard of care.
• Demonstrate a strong understanding of complex medical diagnoses, treatment modalities, and relevant contraindications/precautions while formulating patient-centered therapy plans.
• Implement therapy interventions effectively, focusing on early intervention strategies and same-day evaluations whenever possible to enhance patient outcomes.
• Monitor and manage clinical outcomes proactively, collaborating with fellow therapists to address unique cases and ensure continuity of care.
• Streamline clinical workflows to improve patient intake, referral processing, scheduling, and overall efficiency of service delivery.

Patient Population / Client Focus

In this role, you will work with a diverse patient population within the home care setting. This includes patients recovering from surgeries, rehabilitation after strokes, or those managing chronic conditions requiring ongoing support. Your approach will be holistic, focusing not just on physical recovery but also on improving patients' overall quality of life through tailored rehabilitation programs.

Qualifications

• Valid and active Physical Therapist licensure in the state of Florida.
• A Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited institution.
• Exceptional communication and interpersonal skills, capable of building trusting relationships with patients and their families.
• Strong organizational skills with a knack for multitasking in a fast-paced and dynamic healthcare environment.
• Effective problem-solving and critical-thinking skills, demonstrating the ability to adapt treatment plans as needed based on patient progress.

Preferred Experience

• Experience in home health settings or familiarity with providing therapy in patients' homes is highly desirable.

Compensation and Benefits

• Competitive salary range of $120,000 to $130,000 per year, commensurate with experience and caseload.
