Work Environment

The role is based at our outpatient orthopedic office in Immokalee, Florida, where you will collaborate with a diverse team of healthcare professionals to provide comprehensive care to patients. Our facility is designed to create a welcoming atmosphere for patients, featuring modern equipment and dedicated treatment areas. The schedule for this position is established from Monday to Friday, 8:00 AM to 5:00 PM, allowing for a balanced work-life dynamic.

Key Responsibilities

• Conduct thorough evaluations of patients, assessing their functional limitations and disabilities to develop tailored treatment plans that promote recovery.
• Implement evidence-based treatment protocols that aim to improve patient outcomes and adapt your approach based on the patient's progress and response to treatment.
• Maintain accurate and timely documentation of patient evaluations, treatment plans, progress notes, and discharge summaries, in compliance with state and federal regulations.
• Collaborate with healthcare colleagues and participate in interdisciplinary team meetings to coordinate care and enhance treatment strategies.
• Recognize and document any changes in a patient's condition, adjusting treatment plans as necessary to address evolving needs.
• Engage in discharge planning and ensure follow-up care is appropriately arranged to support ongoing recovery.
• Participate in required training sessions and keep abreast of best practices within the physical therapy profession, contributing to an atmosphere of continuous improvement.
• Assist in facilitating patient access to services by identifying and removing barriers to care.

Patient Population / Client Focus

As a Physical Therapist at Swift Therapy Center, you will work primarily with patients facing orthopedic issues, including postoperative rehabilitation, sports injuries, and chronic pain conditions. Our clientele ranges from pediatric to geriatric populations, requiring a compassionate and adaptable approach tailored to each individual's needs. You will play a crucial role in empowering patients to regain their independence through rehabilitation and education.

Qualifications

• A Bachelor's degree in Physical Therapy from an accredited program.
• A valid license to practice physical therapy in the state of Florida.
• A minimum of two years of experience in outpatient orthopedic settings.
• Current CPR certification, demonstrating your commitment to providing safe and effective patient care.

Preferred Experience

• Experience with electronic health record (EHR) systems for patient management and documentation.
• Familiarity with advanced treatment techniques, such as manual therapy, therapeutic exercise, and modality utilization.
• Proven track record in patient education and facilitating treatment compliance.

Compensation and Benefits
