Work Environment

In this role, you will work primarily in patients' homes, which allows you to develop meaningful connections with individuals in their familiar surroundings. The home care environment can vary significantly from one patient to another, incorporating diverse challenges and requiring adaptability and creativity in delivering care.

Key Responsibilities

• Conduct thorough evaluations and assessments of patients' physical conditions, including mobility, strength, and functional abilities.
• Create personalized treatment plans that reflect the patient's goals, medical history, and therapy preferences.
• Apply advanced hands-on techniques, therapeutic exercises, and a variety of modalities (such as ultrasound, electrical stimulation, and manual therapy) to address pain and improve function.
• Continuously monitor patient progress, adjusting treatment strategies as needed to ensure effective rehabilitation outcomes.
• Educate patients and their families on effective home exercise programs, techniques for pain management, and strategies to prevent further injury.
• Document all treatment sessions meticulously, ensuring compliance with healthcare regulations and maintaining accurate patient records.
• Collaborate with multidisciplinary teams, including physicians, occupational therapists, and other healthcare providers, to ensure comprehensive patient care.

Patient Population / Client Focus

This position focuses on a varied patient population, including individuals recovering from surgery, those with chronic pain conditions such as arthritis, and patients dealing with balance and mobility impairments. Our comprehensive approach tailors therapy to meet the specific needs of adults and seniors, many of whom live with complex health challenges requiring innovative and compassionate care strategies.

Qualifications

• A Bachelor's or Master's degree in Physical Therapy from an accredited institution.
• Active and current license to practice as a Physical Therapist in the state of New Jersey.
• Board certification from the American Physical Therapy Association (APTA) is preferred.
• A minimum of one year of clinical experience in providing physical therapy services, with a demonstrated ability to manage diverse clinical caseloads.
• Proficiency in using various physical therapy techniques and equipment.

• Outstanding communication and interpersonal skills to effectively engage with patients and their families.

Preferred Experience

• Experience in home health care settings, particularly with geriatric populations.

Compensation and Benefits
