Work Environment

As a Physical Therapist in our home care setting, you will be working onsite, primarily traveling to patients' residences throughout Highland Park and surrounding areas. You will collaborate with an interdisciplinary team to ensure comprehensive care coordination and support tailored to each patient's needs.

Key Responsibilities

• Direct and manage physical therapy treatment plans aimed at preventing disabilities, alleviating pain, and enhancing muscle function while upholding the client's highest level of performance.
• Oversee Physical Therapy Assistants to ensure the appropriate execution of individualized treatment plans.
• Conduct thorough evaluations of patients' functional levels and develop targeted rehabilitation strategies derived from diagnostic assessments.
• Assess and recommend environmental modifications to optimize patient independence at home.
• Provide physical therapy services as prescribed by physicians, including, but not limited to, therapeutic exercises, manual therapy, and physical agents like traction and massage.
• Collaborate with physicians and case managers to update and communicate patient progress effectively.
• Prepare and maintain comprehensive clinical records, including evaluations, care plans, and progress notes, ensuring timely submission as per branch policy.
• Participate actively in interdisciplinary team meetings to strategize and evaluate patient treatments collectively.
• Educate clients and their families in the effective use of therapeutic devices and overall physical therapy programs to promote autonomy.
• Supervise and mentor Physical Therapy Assistants during treatment sessions to ensure quality and compliance with best practices.
• Remain accessible to assist Physical Therapy Assistants while they provide direct patient care.
• Take on additional assignments and roles as directed, contributing to the overall goals and objectives of the organization.

Patient Population / Client Focus

You will engage with a wide range of clients, including elderly individuals, post-surgical patients, and those managing chronic conditions. Qualifications

• A Bachelor's degree in Physical Therapy from an accredited program is required. A Doctorate in Physical Therapy (DPT) is preferred.
• Minimum of one year of experience as a Physical Therapist, ideally within home care or an acute care setting.
• Current physical therapy licensure in good standing in New Jersey.
• Certification in CPR, with a commitment to ongoing professional development.
• Excellent interpersonal and communication skills are essential for effective patient and family engagement.

Preferred Experience

Compensation and Benefits

This full-time position offers a competitive salary ranging from USD 100,000 to 120,000 annually, commensurate with experience.
