Work Environment

This position is based in an onsite home care setting where you'll visit patients at their residential locations throughout New Brunswick. Operating under a flexible schedule, typically from Monday to Friday, 9 AM to 5 PM, you will navigate various home environments, adapting therapy sessions to suit each patient's unique living conditions. Key Responsibilities

• Perform thorough mobility assessments and initial evaluations to identify individual patient needs.
• Develop personalized therapy programs encompassing rehabilitation exercises, functional training, and education.
• Collaborate closely with a team of chiropractors, medical doctors, and other allied health professionals to implement holistic patient care plans.
• Monitor patient progress through regular check-ins and adapt therapy strategies based on feedback and patient response.
• Provide education and support to patients and their families, empowering them to take an active role in their recovery journey.
• Document therapy sessions and maintain accurate patient records according to best practices and state regulations.

Patient Population / Client Focus

You will work primarily with a diverse patient population including individuals recovering from surgery, those with chronic pain issues, neurological conditions, or other physical impairments. Your contributions will be vital in facilitating recovery and improving functional independence, particularly for individuals who might find it challenging to access traditional outpatient facilities.

Qualifications

• Must possess a valid Physical Therapy licensure in New Jersey or be eligible for licensure.
• A Bachelor's Degree in Physical Therapy is required; a Master's degree is preferred.
• Exceptional interpersonal and communication skills, with the ability to engage effectively with diverse populations.
• Strong organizational and time management abilities to maintain efficient patient schedules.

Preferred Experience

• A minimum of two years of experience as a physical therapist is required, preferably in an outpatient or home care setting.
• Proven background in creating and implementing therapy programs that yield positive patient outcomes.
• Familiarity with electronic health record (EHR) systems to document patient care and track outcomes.

Compensation and Benefits

In recognition of your expertise and commitment, we offer a competitive salary range of USD 90,000 to 120,000 annually, commensurate with experience and qualifications.
