Work Environment

This position is primarily onsite, allowing you to engage directly with patients in their homes, which fosters a personalized approach to therapy. You will collaborate closely with an interdisciplinary team to ensure comprehensive care for each patient. Typical work hours amount to 40 hours per week, with variable weekend obligations. Key Responsibilities

As a Physical Therapist at Stone Crest Physical Therapy, your responsibilities will include:

• Conducting thorough evaluations of patients to assess their physical capabilities and limitations.
• Developing tailored treatment plans that address individual patient needs, incorporating the latest therapeutic techniques.
• Implementing and adjusting treatment interventions based on ongoing evaluations of patient progress.
• Maintaining detailed and accurate documentation of assessments, treatment plans, and progress notes in compliance with applicable laws and regulations.
• Collaborating with other healthcare professionals and referring providers to ensure seamless continuity of care.
• Providing education to patients and their families regarding treatment protocols and preventative strategies to enhance recovery.
• Participating in community outreach programs to promote health and wellness, establish connections with potential clients, and contribute to organizational growth.
• Supervising and mentoring support staff, including PTA students and aides, to enrich their development and ensure high standards of care.

Qualifications

The ideal candidate will possess the following qualifications:

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• A valid State License in Physical Therapy.

• Proficient computer skills, including familiarity with electronic medical records and documentation systems.

Preferred Experience

While a minimum of two years of experience as a practicing Physical Therapist is required, candidates with additional qualifications or experience in home health care or outpatient settings will stand out. Experience in developing community outreach initiatives or participating in interdisciplinary team settings is also valuable.

Compensation and Benefits
