Work Environment

This role is primarily onsite, working directly within clients' homes. You will encounter diverse home environments, which presents unique challenges in delivering therapeutic care. Key Responsibilities

The Physical Therapist will have a range of responsibilities including:

• Directing personalized physical therapy treatments focused on preventing disability, relieving pain, and restoring or improving muscle function.
• Overseeing Physical Therapy Assistants to ensure adherence to the developed care plans.
• Conducting thorough evaluations of clients' functional abilities through diagnostic testing and assessments.
• Assessing home environments to recommend necessary modifications or equipment to enhance client independence.
• Administering manual and therapeutic exercises, as well as other modalities such as traction and massage.
• Evaluating, fitting, and adjusting prosthetic and orthotic devices while providing necessary recommendations to physicians.
• Maintaining comprehensive clinical documentation including evaluations, care plans, progress notes, and discharge summaries to support overall patient care.
• Participating in interdisciplinary team meetings to discuss and strategize client treatment plans.
• Instructing clients and families on using physical therapy devices and maintaining their therapy programs.
• Completing functional assessments and revising care plans as required.

Patient Population / Client Focus

In this role, you will work with a diverse range of clients, including seniors, individuals recovering from surgeries, and patients with chronic illnesses. Each client requires a tailored approach that considers their unique needs, goals, and lifestyles. Qualifications

To be successful in this role, candidates must possess:

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• Current and valid physical therapy licensure in the State of New Jersey.
• At least two years of relevant experience as a physical therapist, preferably in acute care or home care settings.
• Current certification in CPR.
• A valid driver's license and proof of automobile insurance.
• Strong clinical skills, along with an ability to think critically and make informed decisions in patient care.
• Excellent communication skills, with a focus on building rapport with patients and their families.

Preferred Experience

While not mandatory, the following qualifications are preferred:

• Experience working in home care settings, with a deep understanding of the unique challenges faced in managing client care at home.
• Proficiency in technology and electronic health record (EHR) systems to streamline documentation and communication.
• A specialization in geriatrics or related fields would be advantageous.
