Work Environment

This role is predominantly onsite, focusing on home care services. Key Responsibilities

As a Physical Therapist, your primary responsibilities will include:

• Consulting with patients to assess their physical condition, medical history, and current symptoms, establishing a rapport that fosters trust and open communication.
• Diagnosing movement dysfunction by using evidence-based assessments and developing customized treatment plans that incorporate therapeutic exercises, stretches, and other modalities.
• Educating patients on proper techniques for executing exercises and movements to enhance their recovery process while ensuring their safety and comfort.
• Implementing therapeutic techniques, including manual therapy and modalities tailored to each patient's needs, to promote healing and functional mobility.
• Guiding patients in using assistive devices, such as wheelchairs or walkers, providing instruction and support during their rehabilitation journey.
• Maintaining detailed patient records to track progress, adjust treatment plans as needed, and communicate effectively with other healthcare providers.
• Advising patients and their families on home exercise programs and additional therapeutic interventions that can be beneficial outside of scheduled sessions.

Patient Population / Client Focus

In this role, you will work with a diverse patient population, including children and adults with various conditions such as post-surgical recovery, sports injuries, neurological disorders, and chronic pain. Your approach will be patient-centered, focusing on individualized care that addresses each patient's unique needs, goals, and challenges, ultimately enhancing their functional independence.

Qualifications

The ideal candidate will possess the following qualifications:

• A Doctor of Physical Therapy (DPT) degree from an accredited program.
• A current valid physical therapy license in the state of New Jersey.
• Exceptional verbal and written communication skills to effectively interact with patients, families, and team members.
• Basic computer proficiency, including familiarity with word processing and electronic health record systems.
• A flexible and agile approach to work, with a commitment to patient-centric care.

Preferred Experience

A minimum of two years of experience in a physical therapy setting is required. Candidates with prior experience in home health or similar environments will be prioritized. The ability to work independently while demonstrating sound clinical judgment is essential for success in this role.
