Work Environment

As a part of our home care team, you will engage directly with patients in their residences, providing personalized care and rehabilitation services. Key Responsibilities

• Conduct thorough evaluations of patients to assess their physical strength, mobility, and functional capabilities.
• Develop individualized treatment plans tailored to each patient's needs, utilizing various therapeutic modalities such as manual therapy, exercise programs, and assistive technologies.
• Administer hands-on treatments and instructional sessions, empowering patients to actively participate in their rehabilitation journey.
• Monitor and document patient progress meticulously, adjusting treatment plans as necessary to meet evolving therapeutic goals.
• Collaborate with caregivers, family members, and other healthcare professionals to ensure a holistic approach to patient care.
• Educate patients and their families on best practices for improving quality of life and maintaining functional independence.
• Stay updated with advancements in physical therapy practices through continuous education, workshops, and professional networking.

Patient Population / Client Focus

Your primary focus will be on patients recovering from various conditions, including orthopedic injuries, neurological disorders, post-surgical rehabilitation, and more. You will work closely with a diverse client base, allowing you to utilize your skills across a broad array of conditions and enhance your clinical expertise.

Qualifications

• Bachelor's or Doctorate degree in Physical Therapy (DPT) from an accredited institution.
• A valid and up-to-date physical therapy license in the State of New Jersey.
• Strong communication skills, both verbal and written, to effectively engage with patients and their families.
• Proficient in using electronic health records and therapy management software.
• Exemplary organizational and time management abilities to effectively handle a varied caseload.
• A commitment to ongoing professional development and high-quality care.

Preferred Experience

• At least 2 years of hands-on experience in a physical therapy role, preferably in home care or similar settings.
• Familiarity with various therapeutic techniques and methods applicable to home care.
• Experience in working with interdisciplinary teams is a plus.

Compensation and Benefits
