Key Responsibilities

• Conduct comprehensive evaluations of patients referred by physicians to design and implement effective treatment programs.
• Employ diverse therapeutic interventions tailored to specific diagnoses, addressing orthopedic, pediatric, neurologic, and sports medicine cases.
• Enhance patient experience by demonstrating exceptional customer service and actively contributing to positive health outcomes.
• Collaborate with a multidisciplinary team to optimize treatment plans and ensure continuity of care.
• Provide skilled, purposeful therapeutic interventions in home settings, adapting techniques to align with each patient's healthcare plan.
• Consult with and educate patients and their caregivers, empowering them with knowledge and skills to aid recovery.
• Achieve and monitor individual utilization goals while contributing to business objectives.

Patient Population / Client Focus

You will work with a diverse range of patients, including children, athletes, and older adults, each with unique needs and recovery goals. The hands-on approach in the home setting allows you to connect deeply with your patients, enabling personalized care plans that foster engagement and motivation in their recovery journeys.

Qualifications

• Must have a Bachelor's Degree in Physical Therapy from an accredited program recognized by APTA or CAPTE.
• Active state licensure in Physical Therapy.
• Current Basic Life Support (BLS) certification from the American Heart Association.
• Effective verbal and written communication skills to facilitate patient education and team collaboration.
• Proficient in word processing and essential computer skills for documentation and patient management.
• A commitment to establishing strong relationships with patients to help them surpass functional goals.
• A positive, empathetic demeanor that fosters a strong connection with patients and their families.

Preferred Experience

• A minimum of 2 years of clinical experience in physical therapy, preferably in home care or outpatient settings.
• Familiarity with evidence-based practices and the latest rehabilitation methodologies.

Compensation and Benefits
