Key Responsibilities

• Conduct comprehensive evaluations of patients, including reviewing medical history, performing assessments, and documenting findings to develop tailored treatment plans.
• Effectively communicate evaluation results and treatment plans to patients and their families, ensuring their involvement in the goal-setting process.
• Implement treatment protocols based on individual patient presentations and continuously adapting approaches as required by evolving healthcare standards.
• Collaborate with interdisciplinary healthcare teams, recognizing and documenting changes in patients' conditions that necessitate adjustments in care plans.
• Manage discharge planning, ensuring follow-up care is appropriately arranged, and maintain detailed documentation of patient interactions and progress notes.
• Exhibit strong time management skills to meet productivity benchmarks while maintaining a consistent schedule for patient appointments and team meetings.
• Engage proactively in department marketing initiatives and community outreach to promote our services and strengthen patient access to care.
• Commit to lifelong learning by pursuing continuing education opportunities and serving as a clinical instructor for physical therapy students, fostering the next generation of professionals.

Patient Population / Client Focus

In this role, you will work closely with a diverse patient population, including individuals recovering from surgeries, chronic illness, or age-related mobility challenges. Our focus on personalized, home-based care allows you to develop meaningful relationships with patients and their families, significantly impacting their comfort and recovery journey.

Qualifications

• A Doctorate degree in Physical Therapy (DPT) is required.
• A valid physical therapy license in the State of New Jersey.
• Current American Heart Association's BLS for Healthcare Providers CPR certification.
• Strong passion for providing personalized healthcare solutions.

Preferred Experience

• Minimum of two years' experience in physical therapy is preferred but not required. We value fresh perspectives and may consider candidates who recently graduated.

Compensation and Benefits
