Key Responsibilities

• Develop and implement individualized physical therapy treatment plans that focus on easing pain, preventing disabilities, and maximizing patient functionality in alignment with prescribed medical guidelines.
• Conduct comprehensive evaluations of clients' physical abilities and limitations through thorough diagnostic assessments.
• Oversee and mentor physical therapy assistants, ensuring that therapeutic strategies are correctly executed and that high standards of care are maintained.
• Evaluate patients' home environments to recommend adjustments that enhance independence and facilitate recovery.
• Deliver a wide range of physical therapy interventions, including therapeutic exercises, manual therapy techniques, and the administration of modalities like traction and massage.
• Collaborate in the multidisciplinary care team, participating in case conferences to refine and update the care plans based on ongoing assessments and patient feedback.
• Maintain accurate clinical documentation, including evaluations, progress notes, and discharge summaries that comply with regulatory guidelines.
• Educate clients and families on rehabilitation techniques and the proper use of physical therapy devices, fostering ongoing engagement in the care process.
• Stay accessible to patients and physical therapy assistants to provide guidance and support throughout treatment sessions.
• Complete additional clinical assignments as needed to further advance organizational goals and improve patient outcomes.

This role primarily serves a diverse clientele, including individuals recovering from surgery, managing chronic conditions, or requiring rehabilitation support following an injury or illness. Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• Current and valid physical therapy licensure in good standing within the state of New Jersey.

• Exceptional communication skills, with a focus on building meaningful relationships with patients and families.
• A strong commitment to clinical excellence, empathy, and proactive patient engagement techniques.

Preferred Experience

• Experience in managing a caseload in a home health setting.
• Background in geriatric care or chronic illness rehabilitation.
• Familiarity with the OASIS assessment tool.

Compensation and Benefits

The salary for this position ranges from USD 100,000 to 120,000 annually, commensurate with experience and qualifications.
