Position Overview

Trust Point Physical Therapy is seeking an experienced and compassionate Physical Therapist to join our team as a full-time professional specializing in home care services. Located in Sayreville, New Jersey, this role offers a unique opportunity to work directly in patients' homes, fostering recovery and rehabilitation in a personalized environment. Work Environment

In this role, you will work onsite in patients' homes throughout the Sayreville area. Our team values a flexible work schedule aligned with patients' needs, typically involving 40 hours per week, including variable weekends. The collaborative nature of our workplace means you'll often interact with other healthcare providers and community services to ensure comprehensive care for our patients.

Key Responsibilities

• Conduct thorough examinations and evaluations of patients with functional limitations and disabilities to develop tailored treatment plans.
• Effectively communicate and collaborate with healthcare providers and interdisciplinary teams regarding patient care and develop a continuum of care.
• Interact compassionately with patients and their families, regardless of cultural backgrounds, while adhering to our high standards of customer service.
• Implement treatment plans, re-evaluate progress, and make necessary adjustments, ensuring documented adherence to applicable laws and professional standards.
• Maintain detailed verbal, written, and electronic communication with the treatment team, referral sources, and physicians, ensuring continuity of care.
• Engage in leadership responsibilities, participating in community outreach, program development, and staff supervision, including PTA and aide management.
• Participate in team meetings and contribute to continuous quality improvement initiatives.
• Foster patient education, guiding patients and their families through recovery plans, discharge processes, and overall wellness strategies.
• Document all patient interactions, treatments, and charges accurately for yourself and any supporting staff assigned.
• Ensure compliance with state licensure requirements and commitment to professional development through ongoing education and training.

Patient Population / Client Focus

Your principal focus will be on patients undergoing rehabilitation for orthopedic conditions, post-surgical recovery, and those with chronic pain. You will work with diverse populations, tailoring your approach to meet the specific needs of each patient, while ensuring that all voices are heard and validated in their treatment journey. Your expertise will be critical in improving patient outcomes and enhancing their overall health and mobility.

Qualifications

• A Bachelor's Degree in Physical Therapy is required.
• A current State License in Physical Therapy.
• Excellent verbal and written communication skills to interact effectively with patients, families, and interdisciplinary teams.
• Proficiency in word processing and electronic documentation systems.
• Commitment to maintaining professional competence and best practices in physical therapy.

Preferred Experience

• At least 2 years of hands-on experience in a clinical or rehabilitation setting, preferably with home care.
• Familiarity with electronic health records (EHR) and telehealth technologies.
• Experience with a diverse patient population and understanding of culturally sensitive health care practices.

Compensation and Benefits
