Move Well Rehab Center in Sayreville, New Jersey, is seeking a dedicated Senior Physical Therapist to join our full-time staff. Work Environment

This full-time position requires working onsite within the home care sector, where you will engage directly with patients in their residences. The schedule for this role is from Monday to Friday, 9 AM to 5 PM. Key Responsibilities

• Conduct thorough assessments of patients' movement and mobility to identify specific rehabilitation needs.
• Design and execute individualized therapy plans that cater to the diverse needs of each patient, ensuring adherence to best practices and evidence-based methodologies.
• Coordinate and collaborate with a multidisciplinary team, including MDs and chiropractors, to develop comprehensive treatment strategies that lead to the best patient outcomes.
• Educate and guide patients through tailored exercise programs, rehabilitation protocols, and home care exercises to promote self-management and encourage recovery.
• Maintain accurate and up-to-date patient records in compliance with healthcare regulations and organizational standards.
• Regularly assess and evaluate patient progress, making necessary adjustments to therapy plans to optimize health outcomes.

Patient Population / Client Focus

The role of Senior Physical Therapist focuses on a diverse patient population, including individuals recovering from surgery, managing chronic pain, or rehabilitation post-injury. Our commitment is to provide personalized support while accommodating the unique challenges our patients face, ensuring they receive optimal care tailored to their specific needs.

Qualifications

• A Bachelor's Degree in Physical Therapy is required; a Master's Degree is preferred.
• Must possess an active New Jersey Physical Therapy license or be eligible to obtain one.
• Exceptional clinical skills along with strong communication abilities, allowing for effective interaction with patients and a collaborative approach with team members.

Preferred Experience

• At least 2 years of clinical experience in a physical therapy setting, preferably within a home care or outpatient rehabilitation environment.

Compensation and Benefits

We offer a competitive salary range of USD 90,000 to 120,000 annually, commensurate with experience.
