Work Environment

This position is onsite and requires providing direct patient care in the homes of our clients across Carteret, NJ. You will be working collaboratively alongside other healthcare professionals, including nurses and home health aides, in a compassionate and dynamic environment. Our multidisciplinary team approach ensures that every patient receives a holistic and comprehensive treatment plan tailored to their individual needs.

Key Responsibilities

• Direct and oversee physical therapy treatment, focusing on maximizing patient performance while adhering to established care plans.
• Conduct thorough evaluations of clients' functional abilities using a variety of diagnostic assessments to tailor personalized therapy interventions.
• Collaborate with physical therapy assistants to ensure the effective execution of treatment plans.
• Utilize clinical knowledge to analyze the client's home environment and provide recommendations aimed at enhancing independence and safety during therapy sessions.
• Administer a diverse range of physical agents, including manual therapeutic exercises, traction, and massage techniques to alleviate pain and restore function.
• Participate actively in interdisciplinary team meetings, contributing insights and updates on patient progress and modifying care plans as necessary.
• Maintain accurate clinical records—completing evaluations, progress notes, and discharge summaries in compliance with OASIS guidelines.
• Instruct clients and their families on implementing the physical therapy program and using therapeutic equipment correctly, ensuring a thorough understanding of the treatment process.
• Prepare and submit timely documentation of clinical orders and payroll according to Nealy Fitness Rehab Center policies.
• Support and mentor physical therapy assistants, providing guidance as needed while adhering to applicable state laws regarding supervision.
• Adapt and revise treatment plans based on continuous assessment of client progress and feedback.

Patient Population / Client Focus

The focus of this role is on diverse populations requiring home healthcare services, including elderly patients recovering from surgery, individuals with chronic conditions, and those greatly impacted by disability or mobility challenges. As a Senior Physical Therapist, you will advocate for each patient's unique needs, ensuring they receive individualized attention and resources to enhance their rehabilitation outcomes.

Qualifications

• Doctorate in Physical Therapy from an accredited program.
• Current licensure as a Physical Therapist in New Jersey in good standing.
• Current CPR certification and valid driver's license with automobile insurance.
• Strong communication skills, able to engage effectively with patients, families, and interdisciplinary team members.
• Positive attitude with a strong commitment to clinical excellence, integrity, and a passion for patient-centered care.

Preferred Experience

Preferred candidates will have a minimum of one year of experience practicing as a physical therapist, ideally within an acute care or home care setting. Proficiency with federal and state regulations governing home healthcare, as well as a demonstrated ability to lead and mentor junior staff, will be considered an asset.

Compensation and Benefits

The position offers a competitive salary ranging from $100,000 to $120,000 per year, complemented by a comprehensive benefits package.
