Work Environment

This position is located onsite within our well-equipped rehabilitation facility in Fords, NJ. You will be part of a collaborative interdisciplinary team, working closely with other healthcare professionals and support staff to deliver comprehensive treatment protocols.

Key Responsibilities

As a Physical Therapist, you will:

• Direct and implement individualized physical therapy treatment plans aimed at preventing disability, alleviating pain, and restoring functional mobility.
• Supervise and guide Physical Therapy Assistants in their treatment methodologies to ensure alignment with the established plan of care.
• Conduct comprehensive evaluations of client functionality through diagnostic testing, assessing muscle, nerve, and joint capabilities.
• Evaluate each patient's home environment, providing recommendations that facilitate independence and enhance daily living.
• Administer therapeutic exercises, including instructing and motivating patients in both manual and non-manual exercises.
• Utilize physical agents such as traction and massage in accordance with physician directives to optimize rehabilitation outcomes.
• Evaluate, adjust, and fit necessary orthopedic devices, ensuring compliance with physician recommendations.
• Maintain accurate and timely clinical documentation, including initial evaluations, care plans, progress notes, and discharge summaries.
• Engage actively in interdisciplinary team meetings to collaboratively plan and evaluate patient care strategies.
• Educate patients and their families on physical therapy techniques and device utilization to support ongoing treatment at home.
• Complete functional assessments during home visits, revising plans as necessary based on real-world challenges.

Patient Population / Client Focus

Your caseload will include a diverse array of patients requiring rehabilitation for various conditions, including post-surgical recovery, neurological impairments, and ortho-rehabilitative needs. By focusing on evidence-based practice and individualized attention, you'll contribute directly to improved patient outcomes and satisfaction.

Qualifications

Qualified candidates will possess:

• A Doctorate in Physical Therapy (DPT) with a current, valid PT license in good standing in New Jersey.
• At least two years of experience as a practicing Physical Therapist in acute care or home health settings.
• Current CPR/AED certification.
• A valid driver's license and current automobile insurance, as home visits will be part of the role.
• Strong communication skills, with the ability to work effectively in a team-oriented environment.
• A commitment to ongoing clinical education and personal growth within the field.

Preferred Experience

While not required, preference will be given to candidates with:

• Experience in home healthcare settings.
• Specialized training in manual therapy techniques.
