Century Impact Orthopedic Physical Therapy is seeking a dedicated and qualified Senior Home Care Physical Therapist to join our dynamic team in Carteret, New Jersey. Work Environment

You will be working in a home care setting where you will provide essential physical therapy services directly in patients' residences. This onsite environment allows you to observe and interact in real time, tailoring your methods to each patient's unique living conditions. The schedule for this role is Monday through Friday, giving you the opportunity to establish a consistent rapport with your patients while supporting their recovery journey.

Key Responsibilities

• Direct physical therapy treatment aimed at preventing disability, relieving pain, and restoring muscle function in accordance with individualized care plans.
• Supervise Physical Therapy Assistants to ensure adherence to established treatment protocols and guidelines.
• Conduct thorough evaluations of clients' functional abilities using a combination of diagnostic tests and assessments.
• Assess clients' home environments to recommend modifications that enhance their independence during rehabilitation.
• Administer therapeutic exercises, manual therapy, traction, and other physical agent treatments as prescribed by physicians.
• Monitor, document, and report changes in patients' conditions and responses to therapy, suggesting adjustments in the treatment plan as necessary.
• Prepare comprehensive clinical records, including initial evaluations, progress notes, and discharge summaries, maintaining accurate client documentation.
• Participate in interdisciplinary team meetings to discuss and develop comprehensive treatment strategies tailored to each client's needs.
• Educate clients and their families on therapeutic techniques, the use of physical therapy devices, and overall care programs.
• Provide guidance and mentorship to Physical Therapy Assistants while ensuring compliance with state regulations.

Patient Population / Client Focus

You will primarily work with a diverse client base that may include elderly patients, individuals recovering from surgery, and those with chronic conditions requiring rehabilitative care. Your approach is patient- and family-centered, focusing on their specific needs and ensuring they feel supported throughout the rehabilitation process.

Qualifications

• Graduation from an accredited Doctorate in Physical Therapy (DPT) program.
• Current Physical Therapy licensure in good standing in the state of New Jersey.
• Current CPR certification, ensuring readiness for emergency situations.
• A minimum of one year of experience in a physical therapy role, preferably within an acute or home care setting.
• Strong decision-making skills and a commitment to clinical excellence and integrity in patient care.
• Excellent interpersonal communication skills, with a focus on engaging and educating patients and families effectively.
• Ability to work collaboratively as part of a multidisciplinary team, fostering a positive environment for care delivery.
• Proficiency with regulations from federal, state, and governing agencies that guide physical therapy practices.

Preferred Experience

• Experience in home health care or outpatient settings is highly valued, showcasing adaptability to various care environments.
• Demonstrated ability to understand and utilize electronic health records systems for patient documentation.
• Prior experience in participating as an active member of case conferences to develop patient care plans.

Compensation and Benefits

This position offers a competitive salary range of USD 100,000 to 120,000 annually, based on experience and qualifications.
