Work Environment

This role is based in a home care setting, allowing you to directly observe and support patients in their everyday environments. The flexibility of this position enables you to choose your own hours, operating primarily Monday through Friday, which helps maintain a healthy work-life balance. Key Responsibilities

• Conduct thorough evaluations of patients' physical capabilities and limitations when treating them in their homes.
• Establish and implement detailed treatment plans that incorporate a range of therapeutic interventions, including exercises, hands-on techniques, and the use of specialized equipment.
• Monitor and document patients' progress, adapting treatment strategies as necessary to help them achieve their rehabilitation goals.
• Collaborate with patients, caregivers, and interdisciplinary team members to educate on the use of assistive devices and effective self-care practices.
• Maintain comprehensive records using our digital documentation systems to ensure accurate tracking of patient care and progress.
• Participate in ongoing professional development through workshops and training to enhance your techniques and stay updated on best practices.

Patient Population / Client Focus

Your work will primarily involve patients recovering from a wide range of conditions—ranging from post-surgical rehabilitation to chronic pain management—across all age groups. Each patient's care plan will be customized to their specific needs, considering not only their physical health but also their social and emotional well-being.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution is required.
• A current State License in Physical Therapy is mandatory, with a Doctorate of Physical Therapy (DPT) preferred.
• Strong verbal and written communication skills to effectively interact with patients and their families.
• Proficient in electronic health records and other software tools for tracking patient data.

Preferred Experience

• At least two years of experience as a Physical Therapist, ideally in a home care or outpatient setting.
• Current CPR certification to ensure patient safety.
• Experience in providing education and training to patients and families regarding therapy procedures and assistive devices.

Compensation and Benefits
